If Ranma could think cleary, he would wonder why he was doing this, even though it felt real good. But he couldn't, due to the previous wishes that were made that basically compelled him to engage in sexual activity, damn the consequences.
For Akane's first sex-related wish, he was basically hyped up on lust, pushing him to engage in hot and sweaty sex with Akane, while his manhood(already big) expanded to dimensions that maade it impossible not to be called big or fat.
For Akane's second sex-related wish, there was a new element added: aggression against the lover. For a long time all his frustrations with the mercanry behavior of the girl now impaled on him was bottled up, lest he incure more debts to said girl. But now with the wish... it was being converted into a very rough lover, in order to fulfill the wish of "screw Nabiki silly". After all, to achieve that, it meant he was to screw with her so bad she's going to pass out from the experience.
