I'm not familiar with the books (never read them), but I did see all eight of the movies, so anything I add is based off of that.
3 is interesting but Im unsure of Bella's ability to carry it off:

Well, it does say in the option "For a Time" in parenthesis, so eventually, someone will notice.
Option 2 would be interesting, with a Bellatrix/Harry with Harry's motives, experiences, interests and friends and Bella's methods, insanity and instincts.
Hermione being his girlfriend, really limits the thing. Weird condition in an otherwise open restart.
I don't know. Wouldn't that potentially make Ron an enemy? You know, since he'd be jealous and everything?
It's because I prefer Harry/Hermione. But if you really want, you can ignore this condition.

Harry having a girlfriend, anyone really, makes sure that the story revolves around the two of them.
Hermione is no more limiting than Ginny or anyone else. I don't mind either way, it was more a surprise at an otherwise open, one basic premise with a weirdly specific (to me) condition.
